PLEASE NOTE: The Upper Darby School District Physical and Dental Reports MUST be uploaded to TADS after they have been completed. These forms are only required for children entering specific grades.
PLEASE NOTE: If your child requires the use of or access to ANY medication or medical devices (EpiPen for allergies, inhaler for asthma, glucose monitor for diabetes, etc.), you MUST submit the required forms to the school, including the Upper Darby School District Medicine Form & Consent to Carry Inhaler and St. Bernadette School's Student Medicine Form. If your child DOES NOT require medication or a medical device but DOES have a medical condition that you would like our faculty to keep in mind, please fill out and submit St. Bernadette School's Student Medicine Form.
Please see the CARES section of the website under "Admissions" to complete registration forms for our before- and after-school care programs. Registration and payment is managed by the CARES program coordinator, Mrs. Kathy Mascitelli; her contact information is available in the aforementioned CARES tab.

Your child's teacher will notify you if their classroom requires snacks and lunches that follow an allergy-friendly food plan (ex. nut-free, dairy-free, etc.). You will also be informed whether this affects all food brought into the classroom or only snacks shared with the entire class (ex. treats for birthdays or holidays). We kindly ask that you respect the medical needs of all children in our school and follow the guidelines established by your child's teacher.
